چکیده: 

Efficient distribution of service requests between fog and cloud nodes considering user mobility and fog nodes’ overload is an important issue of fog computing. This paper proposes a heuristic method for task placement considering the mobility of users, aiming to serve a higher number of requested services and minimize their response time. This method introduces a formula to overload prediction based on the entry-exit ratio of users and the estimated time required to perform current requests that are waiting in the queue of a fog node. Then, it provides a solution to avoid the predicted overloading of fog nodes by sending all delay-tolerant requests in the overloaded fog node’s queue to the cloud to reduce the time required for servicing delay-sensitive requests and to increase their acceptance rate. In addition, to prevent requests from being rejected when the mobile user leaves the coverage area of the current fog node, the requests in the current fog node’s queue will be transferred to the destination fog node. Simulation results indicate that the proposed method is effective in avoiding the overloading of the fog nodes and outperforms the existing methods in terms of response time and acceptance rate.

Background: β-thalassemia is the most common monogenic disease caused by abnormalities in the synthesis of the β-chain of hemoglobin. Methods: From January 2018 to September 2018, 90 patients (age >18 y) with β-thalassemia major or intermedia who referred to Rajaei Cardiovascular, Medical, and Research Center, Tehran, Iran, for the assessment of myocardial iron overload were enrolled. All the patients were receiving regular blood transfusions and chelating therapy. Comprehensive transthoracic echocardiographic studies consisting of 2D echocardiography, tissue Doppler imaging, and real-time 3D echocardiography were performed. Results: A total of 90 patients were enrolled in the study. Cardiac iron toxicity (ie, T2* < 20 ms) was seen in 28 (31%) patients; whereas in 62 (69%) patients, the cardiac iron level was undetectable (ie, T2* > 20 ms). Patients with T2* < 20 ms had significantly higher serum ferritin levels than those with T2* > 20 ms (P = 0. 02). No significant correlation was found between the serum ferritin level and T2* (r = − 0. 08, P = 0. 41). The left ventricular ejection fraction was statistically similar in the 2D and 3D examinations. Left atrial endsystolic and end-diastolic volumes were greater in the patients with iron cardiotoxicity than in those with no detectable cardiac iron deposition (P = 0. 01 and P <0. 001, respectively). Left atrial strain was also significantly lower in the patients with critical iron overload. The patients with T2* < 20 ms also had lower left atrial ejection fractions than those with T2* >20 ms, both in 2D and 3D examinations (both Ps <0. 001). Conclusions: Our study showed that changes in the left atrial structure and function precede impairment in the left ventricular systolic function in thalassemia patients with critical myocardial iron loading.

Major thalassemia is one of the hematological diseases requiring multiple blood transfusions, which results in iron overload in the liver, heart and other organs. Current iron chelation therapy consists of intravenous (IV) deferoxamine and oral deferasirox and deferiprone. Although these chelators are effective, many side effects are reported. In the present study, the iron-chelating effect of ciprofloxacin with good oral absorption was investigated.The experiments were done on 60 Wistar rats approximately 250 g. Ciprofloxacin (7 or 14 mg/kg per day) was administered simultaneously with iron (0.03 g/kg per day) or after one-month administration of iron. Ciprofloxacin effect on iron absorption in the liver and heart was studied carefully using atomic absorption.A significant decrease in the liver and heart iron following the ciprofloxacin (14 mg/kg per day) administration was observed, when compared with the control group.This ciprofloxacin-induced tissue iron depletion was more pronounced when it was administered simultaneously with iron, when it was administered for a longer duration (2 months rather than 1 month) and when it was given in higher doses (14 mg/kg per day).Administration of ciprofloxacin may help to decrease the burden of parenteral administration, thereby improving compliance and also the life expectancy of thalassemic patients.
